The Osage Nation Diabetes Program requirements are as follows:
1. You must actively participate in the ONDP for at least 6 months,
by downloading your meter two times in the 6 month period.
2. You must have your Hbg A1C drawn once every three months.
3. You must see your physician once at year.
4. You must see the dentist once a year.
5. You must see the optometrist once a year.
6. You must see the nutritionist once a year.
7. You must have a foot exam once a year.
To qualify for the following services all of the above must be complete.
The services include:
1. Diabetic Shoes
2. Dentures or Partials
3. and/or Eyeglasses.
